If you did not pickup a rebate form at the store, simply enter the four digit rebate number located on your rebate receipt, which is attached to the bottom of your purchase receipt.

I cannot open/view the PDF file
Rebate form documents are in a PDF file format, therefore, to read and print these files you will need Adobe™ Reader™ 6.0 or higher.
The PDF file does not print when I click the "I accept the terms - View & Print Rebates" button
Unfortunately, PDFs do not automatically print when you click the "I accept the terms - View & Print Rebates" button. You will have to go to the new tab or window that was opened or click on the link that shows up to open the PDF file, then click the Print icon in Adobe Reader.
